Opleidersdilemma's bij onvoldoende functioneren van de arts in opleiding tot specialist


Samenvatting

Approximately ten percent of the medical trainees do not graduate. Some of them are dismissed by the training director because of insufficient competencies. The competencies of medical trainees include the roles of medical expert, communicator, scholar, collaborator, manager, health advocate, and professional (CanMeds). Assessing trainees’ competencies for each of these roles is cumbersome. We assume that the process of dismissal of trainees is complicated by the vagueness of the test criteria and the many roles of the training director, who may face a conflict of duties on the interface of healthcare and educational ethics. We are unaware of publications on how to consider this struggle, thus we illustrated these conflicts with two examples from the Dutch conciliation board and described relevant values and mitigating circumstances concerning this decision. We conclude that determining the lower limit of functioning is most difficult with regard to internalized professional values. Furthermore, in cases of insufficient functioning, the roles of coach and assessor should be separated in person to avoid conflicts of interest.
